Step 1: Payment and Scheduling Information.

Payment is collected at the time of treatment and can vary depending on time and complexity of the treatment session. Sessions can be paid for using cash, check, Venmo, Paypal, or credit card (HSA accepted as well).

How to schedule online?

Below find a scheduling buttom that will take you to my squareup scheduling web page. Once there you can select either the Physical therapy Evaluation/Visit or Athletic Movement visit. The difference is in the need. PT Evals are for addressing pain, and disfunction following an acute injury or surgery as well chronic injury or chronic pain. The Athletic movement visit is meant for individuals seeking to improve their performance within their specified sport such as running, Cross-Fit, BodyBuilding, Weight Lifting, and Power Lifting.

Please select a date and time that is available. If there are no open scheduling times, please call for accomidations with the number provided below. 

Can I schedule by phone?

As with your first appointment, you can call or text to book your next appointment at (615)240-7962.

What if we are not in your network?

Lifters Clinic PT Specialist is an out-of-network practice. This means that if you have out-of-network benefits some or all of your out-of-pocket costs will be reimbursed by your insurance company or applied towards your deductible.

You also have the option to pay an affordable cash rate.

Why are you out-of-network with some insurance companies?

This ensures that I am able to provide you with a one-on-one treatment session, as I do for all of my patients (special group treatment sessions for family are available).  If you have further questions, please contact us for more information.

Can I bill my insurance for reimbursement of my out-of-pocket expenses?

In most cases YES! Patients such as yourself can get reimbursement for their treatment sessions. It's as simple as printing a claim form off your insurance company’s website, and submitting it in with the receipt that I provide for you. This will have all of the billing and treatment codes your insurance company will need. The amount of reimbursement is completely dependent on your insurance plan. If you will call your insurance company to inquire, they should be able to supply you with your “out-of-network” Physical Therapy benefits. All insurance policies are different, but most patients can expect to be reimbursed 40-80% of their session fee.

Do I need a referral from my physician for physical therapy?

In most cases NO! This is because Tennessee is a direct access state.  This means you can be seen without a physician’s referral for up to 30 calendar days per TN state law.  The only exception is if you are a Medicare beneficiary.  If you are receiving Medicare and need a referral, please let us know and I will assist you with a payment option that fits your specific case best.

    • Mon-Tues: 7:30am-5:30pm

    • Wed: 7:30am-3:30pm

    • Thurs-Fri: 7:30am-10am/6:00pm-7:00pm

    • Sat/Sun: Special Scheduling (per request and confirmation)
